Depends on how its done. If you want to retain the factory shank (like mscott did here) then threading the shank in a lathe and tapping the knob is the only way because of the cut outs in the handle's shank.

Now if you are wanting to replace the shank as well (which is what I have done with the 110 and Axis handles I have made) then I cut down the handle and remove the knob and shank, drill and tap the base that attaches to the bolt, and then cut my new shank and knob all at one time and cut the male threads onto the shank.

There are several ways to skin this cat, as always Mscott turned out a nice handle.
