I'd start by swapping the extractor with one that's known to work properly. Yours wouldn't be the first to have an extractor cause ejection issues.
i have a mod. 12 lrpv,,right bolt ,left port..while extracting as soon as the neck clears the chamber the extractor lets go and the brass just lays in the action..not a big deal at the range but i also use it for stand hunting..
[6mm. br] and thus no 2nd shot if needed..[should have said as soon as neck clears and the brass tilts it lets go]has anyone had this problem,,appreciate any solution you may have,,Rich

Sharp Shooter Supply extractor ejector kit works for me.
install a .140 extractor detent ball
