Have you ever adjusted your accutrigger? One of the reports I've heard over time, is that if the trigger is turned down to its lowest pull weight, you can have that happen. I had that same problem on my 111, but it turned out to be I didn't have enough clearance between my aftermarket pillar bedded stock and the trigger. The Rub was creating just enough resistance to not let the firing pin hit the primer hard enough even though I could see the firing pin mark on the primer.
Does the loaded round that "misfires" eventually ignite if you try it again?
Are you bumping the shoulder back at all when you're neck sizing?
How old are your Primers and is there a chance that they got damp or dirty?
