Extraction problems with FVT

I've cleaned the extractor, replaced the extractor, cleaned the extractor slot in the barrel, and I still have the same problem with fired cases not extracting. What gives?

Have you cleaned the chamber with a brush?

I have. I'm wondering what else I can do. Perhaps I need to remove material from the front of the extractor so that it goes deeper into the extractor cut, and therefore, further over the case rim?

What exactly is it doing?
Is it not pulling the case out at all?
Pulling the case out partway then it drops it instead of kicking it to the side?
Or something else?

It's an intermittent malfunction. It leaves the fired case in the chamber. I can sometimes re-close the bolt and it will pick it up and extract it, but not always.
