If all you did was polish the surfaces, you didn't take enough out. You actually have to trim the corner of the lug abutment to the point that the case does not touch the edges, but rather in the center of the raceway. ( I tried to take some pics, but my flash wouldn't work). Here is a pic from old files, maybe you will get a better understanding.

I was a bit worried about removing metal in places I shouldn't. I started with sander drum in the Dremel, but quickly changed to the polishing cone.

It seems like it was just popping free (before the mods mentioned above) once it gets all the way over to the side... beyond the point where it would be hitting that corner. Not sure.

Well at least you got it fixed for the time being. Be sure and make a note somewhere that this rifle has a specific mod to the plunger, or you'll be like me and forget just exactly which rifle it is that requires that .125 ball in the extractor and will not work with a .140 lol
