Problems loading Model 16

My new Model 16 in 204 ruger is finally broken in. Now that I can have some fun at the range, I am having problems loading the 4th and fifth cartridge. I am not sure if its a magazine issue or loose bolt issue. Has anyone else had this problem?

You have to be careful how they stagger into the mag. Only accepts 4 on large calibers.

I think I figured it out. Some of the plastic parts in the mag were loose and the cartridge was sitting too far back and the bolt was not picking it up. I placed a small spacer at the back and everything seems to work just right.
