When sites talk about timing the brake on the barrel are they simply referring to which way the ports are pointed?

Yes. Usually it involves machining a measured amount off the barrel end of the brake or adding shims to get it to line up the way it should.

I wouldn't recommend doing any porting where the rifling is in the barrel. screw on ports do the job after the bullet has left the rifling. if you port in the rifling you will loose muzzle velocity.
